Membership Application
Payday advance companies as well as vendors and suppliers to the industry are required to complete one of the following member applications to be considered for CFSA membership.
If you are the owner of one or more payday advance stores, please complete the 2009 Membership Application. In addition, storeowners must submit a CFSA Census Form and Best Practices Acceptance Form with the member application.
Vendors and suppliers should complete the Affiliate Member Application.
